Keep patient information out of general learning workflows
Cohen Coaching is a dental-professional learning platform. Its general course, event, support, upload, and AI workflows are not intended to receive protected health information (PHI) or other patient-identifying information.
De-identify educational material before upload
Remove patient names, contact information, dates, record numbers, recognizable images, and other identifiers before submitting a case example or educational source. If the material cannot be safely de-identified, do not upload it.
Written authorization is required for a healthcare-data workflow
Do not treat the general platform, this notice, or a technical security control as a representation that a particular HIPAA-regulated workflow is authorized. Any such use requires separate legal review, written agreements, approved configuration, and explicit operating instructions before patient information is processed.
Report concerns promptly
If patient information may have been submitted accidentally, stop using the affected workflow and contact privacy@cohendentalcoaching.com with enough detail to investigate. Do not include additional patient information in the report.
